Crest ProHealth is offering a "Better Dental Check-Ups Guaranteed" program. Print the required form here. Basically, you have to visit the dentist by 3/31/10. The dentist or hygenist fills out the form. Then you buy Crest ProHealth products and use them for 6 months. You go back to the same dentist by 9/30/10 who completes the assessment form started on the first visit. If your dentist doesn't see an improvement, then Crest will pay for the purchased products. The products include Crest Pro-Health Toothpaste, Crest Pro-Health Rinse, Crest Glide Floss and Oral-B CrossAction Pro-Health or Oral-B Pulsar Pro-Health or Oral-B Rechargeable Toothbrush Heads or an Oral-B Rechargeable Toothbrush.
