There continues to be confusion about the 4 different Coors Light forms Print the forms All four forms are on this same link in pdf format. When you click it, you will only see the first form which does require a beer purchase. Use your scroll bar and you will see that there are 3 more forms under the first one. Notice that they all have the same box number: 9006, but different DEPT numbers. Dept 101058 and 101060 require a beer purchase. Dept 101059 and 101061 don't require a beer purchase. The state restrictions are also different - see the fine print. That is because some states, like Utah, have laws against requiring people to buy alcoholic beverages.
