There was supposed to be a Prevacid mail-in rebate form in today's paper. I didn't get it.
You can print it here. Purchase dates are 9/18/11 - 11/18/11. Your refund will be less the $4 coupon they assume you’ll use. The $4 coupon is also available on the website. Fine print ---

Offer based on Average Base Price for Prevacid®24HR 28ct according to Information Resources, Inc. Rebate request must be postmarked by 11/25/11 and received by Novartis Consumer Health by 12/2/11."
