The Gap, Spend $15, Get $12 Refund Via Lucky MIR

When you spend $15 at the Gap in-store, you get a free Lucky magazine subscription worth $12. But you can opt for a  $12 refund instead. Read about it here. After you shop in-store, you'll get a card with a unique code on it. You enter the code online here. At the bottom of that form, it says:


"If you do not want a subscription to Lucky magazine and prefer instead to receive a refund, please fill out the form above and mail it along with proof of your GAP qualifying purchase of $15 or more within 30 days of making the actual purchase to: Conde Nast Special Programs, GAP, Box 37722, Boone, IA 50037-0722. Offer valid until June 10, 2011. While supply lasts."
