This is good through Oct 31. You'll need to purchase Pantene products at Walmart, then go to a salon to get the hair cut. Staple receipts for Pantene (purchased at Walmart), and a receipt for the salon charges onto the back of the submission form. Mail these along with the dry ponytail.
