Print the form for a $5 rebate on 1 Lysol product and a flu shot. Receipts must be dated 10/1/11 - 2/29/12. Send original dated receipts for the Lysol product and the flu shot along with the form. If you're getting a flu shot anyway, the $5 rebate would more than cover a single Lysol product.
