Get a free Arm & Hammer Pet ID Tag for 2 proofs of purchase of Arm & Hammer Clumping Cat Litters between 1/1/11 - 12/31/11.Fill out the form on-line, then mail it with your cash tapes with prices circled.

No form is needed. It says at the bottom:
If you do not have a printer , please print the required information onto a 3" by 5" card and mail along with your proof-of-purchase.

1. Purchase any (2) TWO ARM & HAMMER® Clumping Cat Litters between 1/1/11 and 12/31/11.

2. Circle the cat litter and corresponding prices on your original cash register receipt(s).
3. Follow instructions for completing your order form on the right, and press the Register and Print My Order Form button. Your order form will be displayed.
4. Print the order form on your printer using the "Print my Order Form" button.
5. Mail the completed Order Form showing your order number and sales receipt(s) to:
