New date: Print the form for a $10 rebate wyb $30 worth of Olay products (excludes Pro-X Cleansing Device and refills). All purchases must be made by 12/31/11, and postmarked by 1/14/12. Send the form, original dated sale receipts w/purchases circled, and write UPCs on form.
