$12 Refund or Free Subscription P&G Products

Print the form for a free subscription to your choice of Allure, Glamour, Lucky, Self, or Teen Vogue for $30 in P&G purchases from Pantene, Olay, Secret, Venus or CoverGirl. If you already subscribe, it will be added to your current subscription. If you don't want the subscription, you have the option of receiving $12 cash instead. After you purchase the products, complete the form, click submit, print, then look at the bottom of  the second page. Complete the bottom part of Page 2 and write "REFUND" at the top. Then print and mail along with your CRT within 30 days of purchase to Conde Nast Special Programs P&G
