Print the form for up to a $5 rebate on ZzzQuil. Send form, UPC written on form, and original dated sales receipt. Purchase 2/28/16 - 3/13/16. This is the same form that is on the Walgreens website walgreens.com/zzzquil, but this one isn't tied to a store. I don't think you could do both.
