Go here to read about and submit a claim for Tom's of Maine. You must have purchased at least one Tom's of Maine product from Mar 25, 2009 - Sept 23, 2015. The list of products is available at the same link, and include toothpaste, deo/antip, soap, sunscreen, diaper cream, body wash, shampoo, hand/body lotion, lip gloss/shimmer, lip balm, mouthwash, or any other personal or oral care product. You are eligible to collect $4 for each product purchased, up to 7 ($28 total). Must list the products purchased, location, and number purchased, on claim form. Submit by 5/7/16.
