Print the form for a $5 rebate on a 30 count pack of Joint Juice purchased from a club store (Costco, Sam's, etc). Send form, original UPC and dated cash tape from 1/15/15 - 1/31/16.

If you want to purchase a smaller size of Joint Juice, you can use this coupon at the grocery store. A 6-pack at $2 at Walmart is $4.42, so with the $2 coupon, it would be $2.42.
