Find a store that is participating here. For my zip, I see Shopko, and a bunch of independent IGA stores. Notice that there are different participating DATES for each store. My closest store will have the deal 10/3 - 10/16. I'll expect the form to be in their ad during that time period. Limit one per household. (Thanks Joanie B)
