Participating stores: KMart, Marc's, Publix, Shopko, Food Lion, Albertsons, Brookshires TX (10/6 - 11/2 only) and others. At Giant Eagle and HyVee, the form prints out at checkout when you buy enough qualifying products.
NOT participating: Target, Walmart, Walgreens, CVS, RiteAid, Kroger, Marsh, Meijer
If you call 866-315-2203, they'll ask what stores you have in your area, then will tell you which ones participate. Please share whatever info you discover.
